Should I uninstall My Movies 4 before upgrading to My Movies 5?
Can both 4 and 5 be installed on the same PC as two separate programs?

I'm still running My Movies 4.07 myself. I've installed My Movies 5 before, but never on a system that also had My Movies 4 installed. That being said, My Movies 5 uses the same folder structure as My Movies 4 so I'd imagine My Movies 5 will replace My Movies 4 when installed. My Movies 5 also uses a more recent version of Microsoft SQL. My recommendation would be to create a local backup of your My Movies 4 database, uninstall My Movies 4, uninstall Microsoft SQL, remove any My Movies and Microsoft SQL folders left behind, install My Movies 5, then browse to the backup of your local My Movies 4 database from My Movies 5 performing a restore. Thanks for the info... how are you able to still access the collection manager for My Movies 4.07? When I try I get the database error message which blocks me from accessing the collection manager even though it's a standalone install.

I'm running stand-alone installs of My Movies 4.07 on two different systems. I'm always able to access the Collection Management application. I've never had any issue with the installations. Well, from time to time when performing a restore from the installation on my HTPC the database will become corrupt. In fact, it just happened the other day. I was able to wipe out the My Movies and SQL installations, reinstall, then successfully restore my database. No issues. Sounds like your My Movies 4 installation is not in good health. Not sure what to advise short of wiping out the installation and restoring your database, but it doesn't sound like you have a current backup. I backup my database monthly so.
