Yes, I remove categories using the dialog seen in my screenshot above,
then "Save Title" after closing that dialog.
I recorded a log while removing "Scott's never seen" category from
Wonka. I tried to send it using the Debug > Send Log menu item, but
nothing happened, and this notice appeared in the log (even though
logging had been disabled by that time):

I also noticed in Sync.log several recurring errors starting about 1
year ago, all like these. The timestamps all seem to correspond to
when I'm adding new movies in Windows MyMovies.

I will attach the complete log from when I removed "Scott's never
seen" from Wonka. I confirmed that the bug is evident on my iPhone,
which still shows this category on that movie even after sync. (Since then,
I've added the category back on to that movie.)
